For most, it’s a sharp twinge a couple of weeks before their period. </p><p>Because it’s a women’s health issue, no one really knows what causes it, or why some women only experience it on one side, but it’s probably related to the egg follicle swelling up before the egg is released, or to the actual release of the egg. </p><p>For some women, the follicle keeps swelling for longer than it should, making a small cyst that is more painful when it bursts, which is what happens to me. Every month, it caused me three or four days of agonising pain. </p><p>It took several years to get a proper diagnosis, because they kept ultrasounding me at random times of the month, so I’d have nothing there, then I’d have a 3cm cyst, then a 1cm one, then nothing... and, of course, the medical staff always assumed I was wrong about what had been there the previous time... and then the keyhole surgery showed it wasn’t endo and there were no cysts, so the surgeon (while I was still barely awake from the anaesthetic) just gave me a referral to a pain management clinic and said I needed to learn to live with it... and then the first BCP they tried me on didn’t actually stop me from ovulating, which is “impossible” and just proved that I was wrong with my fixation on it being related to ovulation was wrong... and anyway it’s just a bit of cramping, right?</p><p>(This was all made even harder for me to convince them of because I had a mirena and didn’t have periods, so couldn’t show them a nice chart showing pain however-many days before my period. And of course no blood means there’s nothing at all going on.)</p><p>Eventually I got the attention of a good gynae by starting the appointment with the words “I want a hysterectomy” in my late 20s. She actually listened to me, and read my pages of notes (half my ultrasounds are only in my medical records because she took photocopies from my notes), and went away to do some research before putting me on an unusual formulation birth control pill, taken with no break weeks. </p><p>And just like that the pain magically went away. </p><p>I’m not making this post for sympathy, I’m making it so that more people are aware of the potential problem. So that if your friend, or relative, or colleague, or student, if any woman you know complains of a slight pain a couple of weeks before her period you can say “that sounds like ovulation pain, it’s completely normal”. And if her pain is severe and lasts for days, you can say “I read something about that once. You should speak to a gynae about it, because it’s not normal and it is treatable”.
